Abstract
This concept paper examines what it really means to keep humans in the loop of AI-assisted decision making. It argues that superficial review is not enough; meaningful oversight requires time, authority, context, and the ability to disagree with automated outputs without penalty.

Calls to keep a human in the loop are common in AI governance. Yet in practice, the phrase can become decorative.

If a reviewer has no time, no contextual knowledge, no institutional backing, or no right to override the system, then human oversight exists only on paper. This paper distinguishes between nominal oversight and meaningful oversight.

Meaningful oversight requires at least four conditions:

  1. Visibility — the person can inspect relevant signals and uncertainty.
  2. Authority — the person can change the outcome.
  3. Accountability — responsibility is clear when disagreement occurs.
  4. Support — the institution trains and resources the reviewer adequately.

The paper concludes that keeping humans in the loop is not a software feature. It is an organizational commitment.
